Replace boost::scoped_ptr with std::unique_ptr.

Replace boost::scoped_ptr with std::unique_ptr.

This is a direct replacement, skipping over the chance to clean up APIs at
this point.  It does allow us to drop the compile-time dependency on boost,
however.
